An FBI criminal background check form, otherwise known as ‘Form 1-783‘, allows an individual to obtain a certificate of their criminal history. This is common for federal jobs, appointments, and for adoption. The applicant will need to complete Form 1-783 and have 2  sets of FD-258 Fingerprinting Cards completed.

How it works

Our trained mobile notaries and fingerprint technicians will visit your location to complete your fingerprinting.

We will supply the FD-258 fingerprint Cards and Ink for your appointment and we will handle mailing the documents to the proper government departments. Tracking will be included.
